Currently we are in-network with Delta Dental Premier.
Please call us with your insurance policy if it is not listed above as we can still file your insurance for you. Insurance plans can vary drastically, however, being out of network can sometimes means as little as a $25 deductible.
Financing Available
We accept Cherry and Care Credit financing options. These options often offer 0% interest (depending on credit) and several payment plan lengths. Please ask us about how these services can help with your oral health goals while staying in control of your finances.
If you're interested to see if you qualify, you can complete the Cherry application before your visit.
No Insurance? No Problem!

We know that not everyone has insurance, but everyone needs routine dental care for a healthy smile. Insurance co-pays and premiums are getting higher every year. Plans are changing and getting more confusing and covering less. That's why we offer an in-house dental membership plan. For a low yearly cost, you can get the preventative care you need along with discounts on all other necessary treatments from Dr. Kosha and our team.

2 exams with fluoride treatment
2 routine dental cleanings
2 oral cancer screenings
1 emergency examination
All necessary x-rays
10% off all additional dental treatment*
20% off initial periodontal scaling (deep cleaning)
All rates billed annually
Each Additional Adult Family Member : $349/year
Renewals: First Member $299/year Each Additional $249/year
Less than 2 months can lapse from last date of coverage to be considered eligible for renewal rates
Child rates apply to dependents ages 12 and under.
Additional member pricing applies to spouse and dependents up to age 23.
Discount only valid for services rendered at Martin Family Dentistry and does not apply to orthodontic treatment, whitening services, no show or late cancellation fees, specialist services, or services requiring consultation with outside providers.
*If financing is used the discount will reduced by 5%.
Patients with current dental insurance are not eligible.
Membership fees are non-refundable should you not opt to use your second cleaning visit.
